Carriage Hill Assisted Living offers all-inclusive pricing for the Amenities and Services listed under Amenities/Services based on a two-tier level of care system. Both tiers include medication administration. A medical record review and/or face-to-face evaluation of each applicant will be completed by a member of the clinical and/or management team to determine the entry tier level and appropriateness for admission. All evaluation results will be discussed with the applicant and family members, as appropriate. Carriage Hill Assisted Living is not appropriate for applicants who are non-ambulatory, significantly behavioral due to a cognitive diagnosis, or exit-seeking.
