Carriage Hill Assisted Living offers all inclusive pricing for the Amenities and Services listed under Amenities/Services based on a level of care tier system. The following criteria is used to determine the tier level that is most appropriate for each applicant from a clinical and safety perspective for Tiers 1-3. Tier 4 is for residents with moderate to high memory and/or cognitive impairment or a significant medical issue requiring constant nurse monitoring, regardless of the level of assistance needed with Activities of Daily Living. A face-to-face evaluation of each applicant will be completed by a member of the clinical and/or management team and approved by the Director of Nursing. This information is used in determining the level of care that is most appropriate to ensure Carriage Hill Assisted Living will meet the needs of the applicant. All evaluation results will be discussed with the applicant and family members, as appropriate.

Resident’s care is completely within the “Independent/Verbal Cueing/Stand-By Assist of One” level as outlined below.
Resident’s care is within the “Hands-On Assist of One” level in 1-4 areas as outlined below.
Resident’s care is within the “Hand-On Assist of One Staff” level in more than 5 areas as outlined below or has medical issues requiring active monitoring and intervention by nursing staff.
Tier 4 is for residents with moderate to high memory and/or cognitive impairment or a significant medical issue requiring constant nurse monitoring, regardless of the level of assistance needed with Activities of Daily Living.

Carriage Hill Assisted Living NH is not appropriate for residency for any applicant requiring “Complete Care” or “Full Assist of One of More Staff” at the time of admission in any area of Activities of Daily Living as outlined below.
